For the employee’s initial access request, the employee will be required to log back
into the Access Request application to sign the confidentiality statement.
An email will be sent to the employee informing them that they need to sign their
confidentiality agreement.
All access requires a confidentiality statement signature.
Once the employee logs back into the application, the first thing they will see is
the option to view the confidentiality agreement. Click the ‘HERE’ link to view the
confidentiality statement.
The confidentiality statement will be displayed to the user. Click the ‘Sign Confidentiality
Form’ button to sign the confidentiality agreement.
