Overview

You can still authenticate into your account if you have misplaced, lost, broke or left your device at home. Your identity is required to be verified before any changes can be made to your MFA/Duo account. OSU Information Technology (IT) can either add a replacement device onto your account or provide you a temporary bypass code.

 

  1. Open a web browser (such as Microsoft Edge, Mozilla Firefox, Google Chrome, Apple Safari, etc.) and fill out an Identify Verification Form (IVF) with one of the below options.

    1. Online Form (If you fill the Online Form out, in the Additional Information box, please submit if you would like a different device added to your account, or you would like a temporary bypass code.) A ticket will automatically be generated for you.

    2. PDF Form (If you fill out the PDF form, please document on the Reason for Verification if you would like a different device added to your account, or you would like a temporary bypass code.) You will also have to submit this form to the Helpdesk by emailing the form to helpdesk@okstate.edu or by faxing the form to 405-744-7861.

  2. After you have successfully submitted an Identify Verification Form (IVF), OSU IT Security will contact you to inform you of your device being registered to your Duo/MFA portal, an Activation Link for Duo Mobile by SMS/text message, or with your temporary bypass code.
