You may purchase a Hardware Token (Fob) at the University Bookstore located in the Student Union on the Stillwater Campus or from the University Store Online.

 

After purchasing a Hardware Token, your identity is required to be verified before any changes can be made to your MFA/Duo Account. OSU Information Technology (IT) provides a few different options to verify your identity.

 

1. On the OSU Stillwater Campus, visit 421 Classroom Building.

   1. Your identity will be verified in person by an IT employee.

   2. After your identity has successfully been verified, the Hardware Token will be added to your MFA/Duo Account.

2. Contact the IT Helpdesk at 405-744-4357 to verify your Phone Password/Challenge Secret over the phone or by requesting a video call to verify your identity.

3. Open a web browser (such as Microsoft Edge, Mozilla Firefox, Google Chrome, Apple Safari, etc.) and fill out an Identify Verification Form (IVF). PDF forms are no longer accepted for identity verification.

      1. Select the Add Hardware Token (Fob) to Multifactor Authentication Account under the Reason for Verification

      2. Provide the number on the back of the Hardware Token in the Additional Information field.

      3. After you have successfully submitted an Identify Verification Form (IVF), OSU IT will contact you to inform you of your Hardware Token being registered to your MFA/Duo portal.
